What about when someone rapes her with some item causing internal damage...? What about when she has an appendicitis or gallstones or any other physical ailment diagnosed by pain...?

Yeah until you run into real pirates in the Caribbean or Philippines or most of South East Asia. Need a dentist or have appendicitis and those are the simple things. Medical stuff:
